Need to add a Custom Submission Form to a page in Dawn Theme

Can I create a custom form to a page in the Dawn Theme or do I have to download another app? Wanting to add it to this page which is a sub page under Size Advice from my header. Its aim will be for customers to submit 4 x measurements , name and email . And in return we will assess and reply with their best size for purchase. On this page we would also like to add a diagram and some instructions on how to measure the body correctly, with a bit of info about the service we are providing and the guarantees we will give. Im hoping not to have to add another app as my page is running slow at the moment.

you can use the contact form template to that page too but its better to use third party app for providing different information to the customer instead of simple contact form information.
