I don’t know of apps but I guess this can be custom coded as well so html, css, js way.
It’s complexity will depend on how much of the functionality do you want to copy and how much data you have to show for the selection. Also if you want those add-ons features as well that shows once the three drop downs are selected.

Should you wish to create a Pricing Calculator that works like the example you shared, you can try Easify Product Options. The app supports Conditional Logic, allowing you to display the appropriate options and pricing based on each step of the customer’s selection — very similar to how the calculator on the right functions.
Here’s an example I’ve created so you can see how it works:

To begin, you create your options using Dropdown fields. Inside each value, simply enter the corresponding price in the Price field. This allows the total cost to update automatically based on the customer’s selections, functioning just like the calculator on the sample website you shared.
Next, to make the calculator work step by step and only show the relevant options, you can go to Advanced Settings and enable Conditional Logic. There, you can set up rules such as: when a specific breed is selected, only the corresponding services are shown; when the customer chooses “Yes, I want add-on”, the add-on list becomes visible; or when a certain service is selected, only the appropriate weight ranges appear. Thanks to Conditional Logic, the entire flow will behave just like the multi-step calculator you’re aiming for — and you can achieve all of this without touching any code.
Finally, all of these settings can be configured quickly within Easify Product Options without any code changes. You simply set things up according to your needs, and the app will automatically handle price updates, show or hide fields, and calculate the total cost for you.
