Setting Up Questionnaire on Product Page Before Purchase

Hi everyone,

I’m a bibliotherapist, and I’m trying to create a unique product page on Shopify for a “Book Prescription” service.

Here’s what I want to achieve:

  1. The product page should feature a questionnaire that customers must complete before they can purchase the product.
  2. I need the answers to be accessible to me after the purchase is completed.

I’ve read this issue: https://community.shopify.com/topic/1801118 (here is another product which is “in stock” so you’re able to see it: https://lloydspharmacy.com/products/canesten-thrush-oral-capsule-150mg-capsule?variant=39727534604351 ) and I think I need the same process, except I don’t need to “approve” anything… Therefore, i don’t need the “draft” part. Just the questionnaire before purchase.

I’m unsure how to set this up. Does anyone have recommendations for apps, themes, or custom code that could make this possible? Any guidance would be greatly appreciated!
