Shopify Email and SMS Subscription on Customer Account Page

Hi community, I want sms and email subscribed through Customer Account Page.

Here is Signup Form:

I want when the customer enters sign up and enter the phone number then subscribed to sms. Email subscribed logic works but I don’t know how to implement it.
