Everyone who is having this problem check your notifications box (right upper corner in your shop admin) and see if shopify is asking you to verify documents (id). if you don´t verify, everything will appear as it should be activated but simply doesn´t show at checkout. Shopify is not doing it´s job, that sort of thing should appear as a highly visible alert, not just a simple notification you can easily ignore.
