Subscriptions, Add Ons and Customer Accounts Oh My!

I am creating a site for a client that sells a subscription book for kids. When the customer purchases one of the subscriptions (month to month, 5 months or 14 months) we want them to be able to also fill out a short couple of questions so we know how many kids they have at home and their ages. This will dictate how many trinkets and WHAT trinkets to send the customer with their subscription. We also want them to be able to add on an additional trinket if they like (for a fee). And of course, be able to log in to their account at any time and update their address, change amount of children in the household, etc.

Currently, I am using Customer Hub and Seal Subscriptions. However, I am not so certain these are my best options to accomplish what the client wants.

Please help me identify the apps I’ll need (of course being sure they integrate) to satisfy the needs of my client. Thanks!

Thanks Josiah. Can you tell me where can the form live and at what point in the customer journey on a website? Any time? At checkout only? After purchase? We would want it during their experience on the product page or after they choose the product. Please provide any details you can so we understand, thanks.

@brown8075 Our forms can be set up to use the main registration page or using an app block if you want to use it on any page that supports sections (which should be all of them if your theme is OS2.0 capable). If your theme supports app blocks, you can use our app within the product form blocks in the theme editor.
