Problem is it’s old code and very buggy. We would like to REPLACE with an APP. Being able to choose flavors and quantities that always add up to 12 before being able to add to cart is imperative.

The MultiVariants - Bulk order app can be the possible solution to your requirement. MultiVariants app has this feature where you can select multiple variants at a time and also can set restrictions on quantities to your product.
From your given example what I understand that you want to set a product in such a way that your customer can select any variant of your product and they can only choose their 12 desired variants at a time from your product.
So for that, from your Shopify backend, you need to set up your product and create variants under the variants. And then, you need to apply the MultiVariants - Bulk order app. And set the minimum restriction as 12 and the maximum as 12. So, for going to the cart page, your customer must pick 12 quantities from different variants otherwise the “Add to cart” button won’t be activated.
By applying, you can display your variants in different styles like select, list, grid/box, and swatch.
Additionally, through this app, restrictions for minimum-maximum quantities, minimum-maximum order value, predefined bundle, interval quantities, and limits on variants can be applied.
Also, you can set min/max restrictions on any specific product or group of products in your store. You’ll get 3 days of trial for going through all the available features.
