What's the best self-manageable subscription app for online stores?

Hi! We are interested in installing an app on our Shopify store where patients can re-purchase their products on auto-renew. However, this needs to be an app where they can manage their own subscription (change the interval, cancel, etc.) as to not involve or overwhelm our staff.
