The “How did you hear about us?” page isn’t a native feature of the default Shopify form so I just wanted to gather a bit more context so we can get this sorted for you.
In order to edit anything on the native Contact Us page, you would need to :
Click on Online Store > Pages.
Navigate through to your page labeled “Contact Us Page”
This is the section you would usually edit for any other information outside of the “Name, Email and Message” fields.
Do you currently have any apps on your store that you currently use to edit this Contact Us form?
