Which affiliate link app can generate custom forms for all products?

I’ve searched through the Shopify app store but can’t find an affiliate/referral link app that hits our needs.

What we’re looking for:

  • Customers complete a custom form and an affiliate link is generated for them
  • Affiliate link applies to all our products
  • Affiliate analytics (clicks, sales etc.)

What we’re NOT looking for:

  • Forcing our customers to create an account with the app provider
  • App provider facilitating the payouts for our affiliates

If anyone knows an app that hits these points, please let me know.
