Any recommendations for an app that will apply the “call to order” button to certain products?

You dont need an app to do that. You can do that just my editing the product template.

You don’t need an app for it, you can create a template which will have a form instead of payment options.

Any idea what I need to edit to remove the buttons and just show contact us to place order?
These are settings variables that are required for the site/template. If you can tell me which theme you are using, I might be able to provide you with the required steps to make this change.

As this is a premium theme which i do not have access to, I will not be able to help you on this. If this was a free theme like “Dawn”, i would have added the feature on my test store and provided you the solution which could have been usefull to others.
