Which app is best for a customized contact form?

Hi, I’m wanting to add a page to our website that will allow people to submit their name and email address, and also a friend’s name and physical address. We then want to automate a thank you email to go to them.

It looks like using an app with a customised contact form would be the best option, does anyone have any recommendations before I take a punt on one?

I don’t think it does, there are plenty of apps around which will fit the bill, just hoping to get recommendations.
