Why aren't all required fields showing at checkout on my site?

For some reason my Shopify site won’t display all of the fields I have marked as required under Settings > Checkout.

As it stands, only:

  • First Name,
  • Last Name,
  • Email, and
  • Password

. . . are being displayed.

On the settings page, I have indicated that Company Name and Shipping Address Phone Number should also be displayed . . . but they’re not.

What I am doing wrong?
