Why can't I see the customer input form in my testimonials section?

Hi guys,

Two things:

1- I have added theme TESTIMONIALS section in home page, but I can’t see customer input form.

2- Would like to add customer review in product page.

Once you Log in to the Admin, then process the following steps:

Install the product review app on the Shopify store and configuration on the current theme.
