I’m trying to get this form to work on my Dad’s counseling business website:

This is a questionnaire created by the World Health Organization to assess someone’s likelihood of Alcohol addiction. The form allocates a score for each response and the total of the score when submitting the form determines the level of addiction likelihood.
Based on the total from the responses the form directs the customer to different pages:
This was working last night and then today I noticed that there is this additional code added to the script:
“// <![CDATA[”
I’m not sure if this new added code is the reason it’s not working but it’s weird that it worked and now it doesn’t.
I’ve tried removing that code and saving it again but it doesn’t work without it either.
Maybe i’m just fixating on this too much - does anyone else see something obvious that I’m missing?
