Tax not applied to Shipping when it should

We have setup our store to "Charge tax on shipping rates". We have added all the Sales Tax IDs for the various states we collect tax in. We have gift cards set to not Tax but everything else should. Our warehouse is based on Missouri.

The problem we're having is, we occasionally are getting orders where the Freight isn't taxed like it should. One order was shipping to Missouri, another to Oklahoma and other states. Orders are for products that should be taxed and technically Freight should be as well.

Any ideas on why this is happening?
