Provide coupon code for filling out a survey and providing email address

Hi, my product isn't launched yet and I'd like to gather consumer feedback. While in the process, I'd also like to collect the person's email and add it to my customer list. When the user is done with filling out the survey I want to give them a coupon code to use for when the product launches, as well as let them know they will receive and email when the product launches.

The above is one of the lead magnets I'm going to put up pre-launch, but I am also thinking about collecting an email for a free pdf guide or a free tool. 

Finally, just a simple coupon code for providing an email.

What are the best/simplest apps to accomplish this?
