Pop Up App - Being Able To Ask A Question And Have Customer Reply To Question As Well As Give Email

Hello.

I have been searching around the app store for a pop up app that suits us but can not find one yet.

We would like to have a pop up with a picture of one of our products, a question (from us) about the product. A customer field to reply an answer and capture email.

At the moment we can not find an app that allows the customer to write an answer, as well as there email address.

We would like to easily change the product and question from time to time.
