Store Pickup App wont properly uninstall.

I have tried uninstalling the Zapiet Store pickup.  It goes away from my App List, however, the code in my site still exists and its disrupting shipping issues.  How do I fully get rid of this??

Most Apps provide documentation about uninstalling.
