Why is my custom app failing to install on an additional store?

Hi, Hope you are doing well. I am a developer who created a Shopify custom app last year and installed it over 2 stores under the same organization after asking for some API access from Shopify and every thing works fine. Now when I and admin try to install it on another one again under the same organization it gives an error page without any information about an error even in the network tab. Can you please help me out I have been struggling with it for more than 2 weeks now. I even checked verbal logs and everything was fine there.

I'm also getting this error. The app has previously been installed successfully. As a side note custom app installation link was expired so a new one was generated before this error started happening, leading me to believe that Shopify is occasionally generating broken urls.

This is no new thing, this github issue explains a similar phenomenon in which a Shopify dev responded and eventually a fix was implemented (but for a different url param)

The client id seems to be correct but I believe the signature is the culprit.
