Why can't I fill out the name field on my store's appeal form?

My Shopify store was recently terminated for seemingly no reason I can think of. The termination email contained a link to an appeal form, so I went to fill it out. For some reason I am unable to fill out the name field (which is required), so I can't send my appeal. Someone else was having a similar issue, and was told to clear his cache and ensure his browser was updated. I did this, but to no avail. I even tried opening the link on different devices, but am always unable to enter my name to complete the form. If anyone could help me with this issue, I would greatly appreciate it.

It's great to know that you have already attempted the troubleshooting steps! Regarding the form issue, have you entered the Account Owner's name correctly? Please ensure that the name matches exactly as it is on file. If you encounter any error messages after entering the name, please feel free to share a screenshot with me here.

Hello! I’m experiencing the exact same issue. I’m trying to submit the form but it keeps marking my name as incorrect. HELP

Hey, I resolved the issue. They aren’t fixing it for some reason. What you
have to do is change your name and stuff on your account, when you do that
it will auto fill for you.

Hey, I resolved the issue for myself. What i did is add my name and stuff on my account in like the settings. When I did that it auto filled for me.

Have you checked that the name you're entering on the form is exactly matching the account owner name that's on file in your settings?

 

As you can see, this seemed to fix @Just1n issue, so I'd reckon trying that first.
