I’m wondering if you’d like customers to provide this information on their customer profile page (the Customer Portal)?
If that’s the case, it’s very possible using an app in the Customer Portal. Here’s a quick video I recently did about that.
This is a no-code solution but using Customer Fields to create a custom form (if you’d like to gather more information than just a birth date, you totally can) and that form would automatically save each answer as a metafield on the customer page in your Shopify Admin.

No, it is an inline form. We do have a pop-up option too. but you mention inline form, so I recommend a inline option
