I want this URL to allow the customer to upload an image that I can then either approve or manually add to the gallery. Is there an app for this? Will this app allow me to have the feed of photos / reviews on a separate page? The previous apps I looked at seemed to only link to the product pages which is not what I want.

Yes I think some Reviews apps are able to install a Reviews Page, where shop-level reviews (not product or order-level reviews) can be collected and contain photos.
