We would like to be able to make reservations for multiple times and link them to customer information.
I also found an app called “BookThatApp”, but it seemed to follow the purchase flow and seemed to be too strict with the above requirements.

Chris here from BookThatApp! The majority (if not all) of the booking apps on Shopify integrate fully with the Shopify order process where the customer chooses a date, adds to cart and checks out. Some apps (including BookThatApp) allow you to book provisionally or pending and let you confirm the booking at a later time but they still go through the Shopify checkout (unless it’s a no charge booking).
If you are looking for an offline booking form, you may be better off going with a form builder app where you can create a custom form that will allow your customer to choose their preferred dates and submit their information. You may want to check out the app store for some suitable form builder apps.

Alternatively you could hire a Shopify Expert and build out a custom page with a static calendar if you don’t need real time availability.
