Add engraving to product and change price if used

Hi guys,

as in the title. I want to add engraving to the product. and add 10$ for example if the user requested engraving.

I saw this on a page, where it is already used.

Somebody knows which app this is ? Or is it some own coding?

If yes, can somebody code this for me too ?
