Can I add custom fields to my checkout form?

Hi all,

I was wondering whether it is possible to add field to the check-out form. I would like to add following field:

  • Sex

  • Birthdate

Stores must be on a shopify Plus plan to edit checkout.liquid

All other plans should collection such information before checkout either on product pages, in the cart, or in customer registrations.
