Collect freight information (add form) to checkout - Minimal Theme

I’m looking to collect additional customer information on the checkout page - is it possible to add additional fields or a secondary form on that page? When a customer orders 3 or more of our workbenches, we have to ship via freight (rather than our usual FedEx Ground). We want customers to be able to specify any special services (ie lift gate, call before delivery etc.) and have that information included in the submitted ordered.

Also, we offer free shipping and create our own shipping labels (not through Shopify or any apps). So, we don’t require the checkout page to calculate shipping rates or shipping labels.

Currently, only shopify plus stores have access to checkout.liquid file which is required to make modifications to the checkout. So if you are not on shopify plus then it is not possible to modify checkout.

As a workaround, I think the required information can be collected conditionally in the cart e.g above checkout button.
