Hi all,
I am working on the password page for my new website and I would like to intergrate a form to collect some data. I have 6 input fields that I would like to include.
I have mocked up a version of the form on the image below (in the red box).
Please let me know if you have any ideas on how I could do this?

Try Shopify Forms?
Then you can go to Customize, select “password page” from the top drop-down and add an “App” section to show the form from the app.
When Form is submitted, it will create a Metaobject with the data in your store, so you should be able to process this data further.
