Can someone help me with how to customize our contact us page and add fields?

I have a little knowledge of HTML and CSS but am not familiar with the liquid language.

Something like this (see attached file)

Hi @NanoSG , it’s described here, I suppose it will help you if you have little knowledge of html and css, however do not forget to duplicate your theme before editing it :slightly_smiling_face:
