To customize the contact form on the Prestige theme, you will need to make changes to the code in the theme files. Here are the general steps you can follow:

  1. Go to Online Store > Themes → Prestige theme → Action button → Edit Code
  2. In the theme files, navigate to the Sections folder and find the file named “contact-template.liquid”.

If you aren’t comfortable with coding, installing a third-party app is the easiest way to add custom fields to your contact form. There are a few apps on our Shopify App Store here that can help you customize your contact form:

Third-party developers make these apps outside of Shopify. If you need more information about the app’s functionality, I recommend contacting the app developers.

If you’d like to test some coding skills, you can look at our Shopify Developers page here on adding some extra codes onto your theme’s coding to add more contact form fields. Keep in mind before you customize a theme, it’s a good idea to make a backup copy so that you can discard your changes and start again if you need to.
