How to obtain Sender and Recipient Contacts at Check out page (Debut Theme)

Hi everyone,

My business involves gifting, as such I need to:

1. get the sender’s Name and Contact

2. get the recipient’s Name, Contact and Delivery Address.

How can I get them in my Checkout page, as I the Debut Theme only has fields for 1 party.
