Looking for a Gift with purchase app BUT with a required discount code

Hello Shopify Community!

I am trying to offer a free gift to our customers that filled out a survey for us, the caveat is we do not have their email addresses to send a link. We are handwriting and mailing thank yous with this free gift but would like for them to choose. I am having a hard time finding an app that allows:

  • me to pick a gift with purchase from a specific collection of items

  • requires a code given to these specific customers

The app must require a code for the free gift.

I’ve tried Easy Gift, BOGO, Free Gift, Koala Upsells, and GiftBox. There has got to be a way to do this!

Please help!
