Want Custom size form in products details page

Hi! I want a custom-size form on the product details page where customers can add their custom dimensions for the clothes.
In the attachment I have added the screenshot, on the click of custom there should be an open form where customer can add their details.

Please guide me on the solution for this. Thanks
