The ‘Contact Us’ page seems to come up with this error (image below) each time it is used. Does anyone have an answer to why this might be happening?

I’m not able to see the image you mentioned in the file you attached below, though. So that I can take a look over this, can you please reply back to me here and embed the image into your response using this button?
That should help ensure the image comes through to me correctly!
In the meantime though, can you please confirm which theme you’re using on your site? You can find this information directly in your Shopify admin under ‘Online store > Themes > Current theme’.
