Why is the wrong product variant being added to the cart?

as per the title,

I have been informed that the incorrect variant is being added to the cart. for example if someone selects a medium blue tshirt, when they go to the cart it has added a small black instead. I am guessing it is reverting to the first available choice in the drop down but im unsure why this is happening.

Also i think it is happening for some but not for others which is odd.
