You need to change the name attributes of the inputs and textarea - remove the translation and set it as the official documentation

Thank you, Ive tried this however I still dont get any result i fill in a test form, and I dont receive any email.

Thank you friend, seems to work now but i think i had an issue with email verification. All good now though!
