I am helping another seller with their store that was set up by someone else. Their previous mod added the relentless variant app and she is now paying $18.99 a month for something she doesn’t need. If I remove the app, will that remove all the variants for all previous products loaded on to her store??

I’d be glad to support you further and help resolve this issue. First, let’s make sure your friend uninstalls the app to cease all future billing immediately. To do this:
From your Shopify admin, click Settings > Apps and sales channels.
In the row of the app that you want to uninstall, click Delete.
In the message that appears, click Delete.
Some apps modify the theme of your online store. You should check to see if the app requires additional uninstallation steps before you remove the app from your store. For further assistance, connect with the App’s Developer directly.
Once that is done, it is safe to assume that any customizations made using that app will be removed. With that being said, I cannot speak on what impact this will have on the store without taking a deeper look at how it is set-up.
Therefore, I advise your friend to connect with our live Support team directly to investigate the back-end to avoid any inadvertent changes.
