I am looking for a way to add an obligatory question in the cart page. The customer have to type in the answer to move to checkout. I also need to have an access to those answers after the fact. I also need a checkbox underneath. I attached the example. I would be grateful for your help.

Thanks for providing the screenshot. I can see how having that comments box and checkbox would be helpful for certain situations. From looking at the screenshot, it’s likely that it is a custom solution that a developer has coded into the cart. However, there might be a few ways you could achieve this by tweaking some settings and installing apps.
Depending on the theme you are using, you may be able to turn on cart notes in the theme settings. This would basically show a text box in the cart that would allow customers to type in information about the order which would be readable by you after the order is placed. You can also change the language of the question to reflect the specific information you need. We have a full tutorial on how to do this here.
For the checkbox, you would likely need to install some sort of app to enable that functionality. The Shopify App Store is a great place to search for all kinds of apps. I had a look and found the following apps that would allow you to add a customizable obligatory checkbox:
One of those apps, combined with the above notes solution may fit your requirements adequately. If you have any specific app questions, you can actually reach out to the app developers directly before installing the app via the support tab on the app page. Also be sure to take note of any applicable app charges before installing.
While looking on the Shopify App Store I also came across this Checkout Comments App. It is a free app that allows your customers to input comments after their purchase. It might be useful if your theme doesn’t natively support cart notes.
