I am a dropshipper (www.lotusmassagechairs.com) and I have some suppliers who offer white glove shipping services as well as extended warranties. I would like to add these options to checkout similar to the picture below. What is the easiest way to do this when I do not want it for all products? It would be fine with me if it showed, but is grayed out for certain products.

There could be two approaches you can follow.
- Using bundle app
You can list your additional services as a different product in Shopify with their cost if any
Use bundle APP available at Shopify and create bundle which will be available to buy together.

Custom code in theme
You can list the service as separate product and we can customise the theme in a way.
We will give option to choose the service in product page in admin.
If product selected then we will show a drop-down at that product page which will have product name in it.
On click of add to cart CTA product will be added into the cart along with service with cost added.
